How this works:

If you forgot your password, then we will lookup your records based on your License #, and send your password to the email address we have on file for you.

If you forgot your License #, then we will lookup your records based on your email address, and send both your License # and your password to your email address.

If you need to update your records and/or correct your email address, please call the Board directly.
